2. Trade union registration:
In case of a company with more than 5 employees:

The company's certificate of business registration (Provided by the customer)

The written employment declaration (approved by the People's Committes of the Disctrict where the company locates)

The list of members of the trade union.

2 2x3 photos for each member.
In case of a company with less than 5 employees:

Ineligible Confirmation Letter for labor union’s establishment

The written employment declaration (approved by the People's Committes of the Disctrict where the company locates)
3. Month salary system registration:
In case of a company with more than 10 employees:

The written decision to establish the company's trade union (issued by the Labor Federation)

The written employment declaration (approved by the People's Committes of the Disctrict where the company locates)

The minutes of meetings of the executive board of the trade union on wage scale

The written provisions of standards of each positions in the wage scale

The company's written rules for employees

The company's written employment agreements

The company's employment contracts

The written wage scale
In case of a company with less than 10 employees:
